Is it actually that bad? Like I get it would be dangerous if someone ate them a lot, but if someone only ate a few a couple of times, would it be OK? Also what are the actual risks?

So the main concern is a bowel blockage called a beozar, which needs to be surgically removed. When indigestible fibers (like hair, some plant fibers, and of course cotton) are consumed, they often don't pass through the body. Now, you want to know the occasional risks rather than chronic. While this is mostly a chronic thing, it actually can become dangerous fast. Or at least really fucking uncomfortable, trust me. I got something similar from bulimia's lovely digestive effects (not a beozar and not indigestible thank god) and ended up in the emergency room because of the pain. Even if I didn't need surgery, I was at that level of pain for two weeks.
Now, if a full out blockage doesn't occur, you can expect some nutritional issues. The cotton will make it harder for your body to absorb important nutrients, at least while its in your intestinal tract.
Another thing is that there are actually toxins in cotton that can make you quite sick.
And of course, there is always a very real choking hazard. Even soaked, the cotton ball poises a risk as it cannot be chewed, and absorbs liquid from the mouth and throat.
